Responsible, resident-centered management of psychotropic medications — reducing dose and dependency wherever it's clinically appropriate, without compromising stability.
Why It Matters
Psychotropic medications play an important role in managing behavioral and psychiatric symptoms in older adults — but they should never be used at a higher dose, or for longer, than a resident actually needs. Our Gradual Dose Reduction (GDR) program exists to make sure every resident's medication regimen reflects their current clinical picture, not just their history.
That means:
Psychotropic medication use is grounded in an appropriate diagnosis of current behavioral symptoms — not a default response to disruptive behavior.
Environmental, psychosocial, and medical causes of behavioral symptoms are considered and addressed alongside — or instead of — medication.
Every resident's regimen is kept free of unnecessary drugs: no excessive dosages, no excessive duration, and no medication continued without adequate monitoring.
How the Program Works
Ongoing evaluation, not a one-time check. Every resident on a psychotropic medication is evaluated by one of our psychiatric providers, then re-evaluated at appropriate intervals — at minimum, quarterly.
At each evaluation, we determine:
Whether the psychotropic medication is still clinically necessary.
Whether the current dose and duration are appropriate, with clear documentation of the behavioral symptoms that support continued use.
Whether a dose decrease, discontinuation, or switch to a medication with fewer side effects is appropriate.
Whether non-medication approaches — behavioral strategies, environmental changes, staff-level interventions — could reduce or replace the need for medication.
New residents and new prescriptions are evaluated as part of the initial psychiatric assessment, not held until the next quarterly cycle.
Every reduction attempt is tracked — whether it succeeds or the resident needs to return to a prior dose — so your facility has a clear, documented history of good-faith, clinically-driven medication management.
Residents are protected from over-medication. The goal is always the lowest effective dose, not the most convenient one for managing behavior.
You get clear documentation. Every evaluation and every reduction attempt is recorded, giving you a defensible record of responsible psychiatric care for your own licensing and quality reviews.
Behavioral support comes first when appropriate. Medication is one tool, not the only one — we look at non-pharmacological approaches before or alongside any prescription.
No added cost. GDR reviews are included as part of our ongoing medication management service and billed through Medicare and Medicare Advantage — not billed separately to you.
